Summary
Requirements
To take the course students must:
· be at least 17 years old
· have a valid, unexpired government-issued photo ID
· a high school OR college transcript (unofficial copy is fine)
· a signed social security card (the name on the card must exactly match the name on the government-issued ID)
· be able to pay for and pass both a background check and a drug test
· obtain required immunizations
· purchase required uniforms and shoes
· purchase books
· MUST HAVE A CLEAR BACKGROUND CHECK AND PASS A DRUG TEST
Students will spend approximately another $350-$400 for background check, drug test, physical, immunizations, books, uniforms,etc.

Please be prepared on your first day of class to have the following:
- Your signed social security card- present to the instructor for verification. This is required by the Department of Health Service Regulation. Students without their social security card will be dismissed from the program.
- Driver's License or state issued photo ID such as a passport or military ID- present to your instructor for verification. This is required by the Department of Health and Human Services. Students who do not present their state issued photo identification will be dismissed from the program.
- High school or college transcript, up to date
- Physical form (see the link above for the CFCC physical form)
- Immunization Information
- Background check confirmation form
- Drug Screen Urine Chain of Custody Form
- Required Ceil Blue Scrubs
- Textbook Hartman's Nursing Assistant Care-Long Term Care 5th edition and the corresponding workbook
